Abstract: Background: Preterm birth, defined as a delivery occurring before 37 completed weeks of gestation, is one of the leading causes of neonatal morbidity and mortality and remains a major challenge in modern obstetrics. Early identification of pregnant women at increased risk is essential for the implementation of effective preventive measures. Objectives: This study aims to develop a clinical and care protocol for ultrasonographic screening to prevent preterm labor at the Maternity School of the Federal University of Rio de Janeiro, integrating the best scientific evidence on diagnosis and management. Methodology: This is a descriptive, theoretical study based on a literature review, including guidelines and recent articles from recognized institutions. Conclusion: It is expected that standardizing care practices will promote the uniformity of maternal-fetal care, optimize health resources, and reduce the incidence of preterm birth, thereby contributing to improved neonatal outcomes.

Abstract: Fetal hydrops is a clinical condition characterized by the abnormal accumulation of fluid in at least two fetal compartments, such as ascites, pleural effusion, pericardial effusion, or skin edema. It can be classified as immune or non-immune, the latter accounting for more than 90% of current cases. This is an ultrasonographic finding of high severity, frequently associated with significant perinatal morbidity and mortality, requiring thorough evaluation and a multidisciplinary approach. The wide range of possible etiologies — including congenital infections, chromosomal abnormalities, cardiac malformations, and genetic syndromes — demands targeted investigation and specialized clinical knowledge. Early recognition of the condition may allow clinical, intrauterine, or obstetric interventions that can influence fetal prognosis. Objective: To report a clinical case of fetal hydrops managed at the Maternidade Escola of the Federal University of Rio de Janeiro (ME-UFRJ) and to review the main diagnostic, etiological, and therapeutic aspects of fetal hydrops described in the literature. Methods: This is a descriptive case report with a narrative literature review. Retrospective data were obtained from the patient’s medical records, and a literature search was conducted in PubMed, SciELO, and Lilacs databases. Justification: Fetal hydrops remains a diagnostic challenge in fetal medicine and requires comprehensive investigation and individualized management. The lack of standardized protocols contributes to unfavorable outcomes. Case reports and literature reviews enhance clinical practice and foster academic and scientific discussions on this complex condition.

Abstract: Introduction: The advent of fetal medicine has enabled the prenatal diagnosis of various congenital anomalies, which represent the second leading cause of neonatal mortality in Brazil. Early identification of these anomalies enhances prenatal care, allows for informed planning regarding pregnancy and delivery, and provides the option to consider pregnancy termination in cases of invariably lethal fetal conditions. Such decisions may prevent maternal-fetal harm when the continuation of pregnancy offers no therapeutic benefit. Objective: To develop a care protocol for managing cases eligible for pregnancy termination due to invariably lethal fetal malformations identified in the prenatal period. Materials and Methods: This study involved a comprehensive review of medical literature, legal frameworks, and ethical resolutions concerning pregnancy termination in cases of invariably lethal fetal malformations. Conclusion: The development of a care protocol is crucial, as the obligation to continue a pregnancy under such circumstances imposes significant psychological distress on the pregnant individual and subjects them to unnecessary risks. The proposed protocol emphasizes the importance of structured care pathways, timely psychological and medical support, reduction of bureaucratic delays, and standardized procedures for decision-making, prescriptions, and follow-up. This approach aims to ensure dignified and compassionate care, representing a significant advancement in patient well-being
