The objective of this study was to evaluate access to social benefits for children with microcephaly, considering the public health emergency of the Zika virus (Espin-ZIKV) in Brazil. Cross-sectional study, with extraction and description (national and regional) of data from the Live Birth Information System (SINASC), Public Health Events Registry Microcephaly (Resp-Microcephaly), and Unified Benefits Information System (SUIBE), between 2013 and 2021. We compared the granting of the Continuous Benefit Benefit/Species 87 (BPC/E87) for microcephaly with the granting of Lifetime Pension intended for cases of congenital Zika (Species 60- E60) and we described the active E60s at the time of data extraction from SUIBE (May,2023). 20,464 suspected cases of Congenital Zika were reported in Resp-Microcephaly during the period studied; 20% of affected patients received some social benefit: 3822 received BPC/E87 and 705 E60. The study showed a national average of eight BPC/E87 for every 100,000 live births (LB) due to microcephaly in 2013, before Espin-ZIKV, and 5 BPC/E87 for every 100,000 LB in 2021 (p<0.01) after Espin-ZIKV. We observed 689 children with active E60 between 3 and 8 years old, 504 aged 7 years old, and of these 454 in the Northeast (May,2023). The study of BPC/E87 and E60 grants for children, possibly with Congenital Zika, showed low social protection for these and a reduction in the average number of BPC/E87 grants for microcephaly in general, after Espin-ZIKV. The high number of active E60, the majority in children aged 7 years, showed important survival rates for these children.
