Objective: This study aims to evaluate appendectomies performed for acute appendicitis at the Clementino Fraga Filho University Hospital from 2014 to 2023 to determine the prevalence of incidental appendiceal neoplasms. Methods: This is an observational, retrospective study involving 106 patients who underwent appendectomy for acute appendicitis during the study period. Clinical and histopathological data were collected from the Medtrak system, excluding cases in which appendectomy was part of a more complex surgery. Results: Of the patients analyzed, 2.83% had incidental appendiceal neoplasms, including neuroendocrine tumors and low-grade mucinous neoplasms. The average age of patients with neoplasia was 54.67 years, with a predominance of women. Among the three patients diagnosed with neoplasia, one died in the postoperative period, and only one of the other two required reintervention with right hemicolectomy, both of which have been monitored in our hospital to date. Conclusion: The histopathological analysis of appendectomies proved to be essential to identify incidental neoplasms, whose high prevalence in our institution may reflect the profile of the patients treated. The absence of a histopathological report in a case reinforces the need to standardize records. Future studies may improve the management of these conditions.
