Relato de caso atípico de Doença de Zuska – Metaplasia Escamosa de Ductos Lactíferos

Resumo

Zuska disease (DZ) or squamous metaplasia of the lactiferous ducts (SMOLD) is an inflammatory process in the central portion of the breast, due to the occlusion of an abnormal duct, through epithelial desquamation process. The main risk factors are smoking (90%), obesity, trauma, diabetes mellitus (DM), iatrogenic procedures and immunosuppression. We presente the case of a 56-year-old patient, with a clinical picture of a cutaneous fistula in the left breast, with resection of the main ducts and a histopathological report of ZD. Our case describes a non- smoker patient, which represents a rare case since the absence of this importante risk fator for ZD. This atypical fact can be justified, in lower prevalence, by having DM and by previous treatment for sarcoidosis with immunosuppressants. Clinical and radiological findings are nonspecific. This report aims to demonstrate the importance of imaging studies, such as mammography (MMG) and ultrasound (USG), which can assist in the differential and exclusion ZD’ diagnostics, to rule out, mainly, the possibility of neoplasic causes of the breast disease.
