For almost a decade, the World Health Organization has stated that all women have the
right to the highest possible standard of health, and this includes having the right to respectful
and dignified care throughout the pregnancy and puerperal cycle, which is also their right be free from violence and discrimination. Therefore, this study's general objective is to discuss, through an integrative review, the dilemmas and determinants of cesarean section in the context of obstetric violence. Materials and methods: this is an integrative review. The search for the material studied was carried out in all databases available in the Virtual Health Library in two stages in the months of August to October 2023. As inclusion criteria: scientific articles available in full and in Portuguese and as exclusion criteria: duplicate texts, integrative reviews and articles that, after reading the abstracts, did not fit the proposed theme. In the second step the inclusion criteria was added: texts published in the last five years. The final sample consisted of 11 articles. Result: the main dilemmas and determinants found were fear of the unknown, fear of labor pain, lack of information, partial information and the influence of professionals. Conclusion: it was possible to identify that there is a lack of knowledge about normal birth and that obstetric violence has different ways of presenting itself and can, directly or indirectly, increase the chances of an unindicated cesarean section occurring where the woman may end up finding herself in a dilemma: A birth with pain, suffering and abandonment or a cesarean section?
