Resumo
This work has as its object of study the accident occurred at the Laboratory of Metallography and Practical Classes at PEMM/COPPE/UFRJ in 2018, which can be considered one of the most serious accidents ever to occur at the UFRJ Technology Center. Serious accidents usually have in their genesis aspects of their own management and dormant risk situations, that perpetuate for years in organizations, until they cause accidents of large proportions and serious consequences, often fatal. The aim of this study is to build recommendations for improving safety in light of the human and organizational factors (FHO) approach. This approach allows shifting the focus, often placed on the individual, during accident analyses. The focus is now on the factors that determine behavior, especially working conditions and the influence of management in carrying out the activity. To analyze the accident, documentary research, interviews, visits and activity monitoring were carried out, in addition to a meeting with the PEMM community. The obtained results were surveys about previous accidents occurred at UFRJ and PEMM; the use of Nital in the practice of Metallography, the main activities of the laboratory and the occurred accident. This research served as the basis for the collective construction of recommendations based on the experiences of technicians and students, discussion about the accident and sharing of the main risks.
